Every requirement in a regulated product traces back to something a real user needed. When that user comes back with feedback, the hard part is reconciling it against the documented baseline you already have: deciding what is genuinely new, what revises an existing requirement, and what conflicts with an already finalized decision. Often that reconciliation, and the writing that captures it, happens by hand, one note at a time, with edits pointing back to the exact statement that justified them.
